Mental Health Evaluation

We are partners in the therapeutic process. Applications of the mental health evaluation are delivered as a means to gather an accurate understanding of your primary mental health concerns and the contributing factors that brought you into treatment. Based on the information you provide and the specifics of your situation, I will offer you education pertaining to your mental health symptoms and a more accurate understanding of what you are facing.

Mental health evaluations are conducted at the onset of treatment and inform your treatment plan, diagnosis, resources, referrals and/or adjunctive services, and all other recommendations I may provide you throughout your treatment.

Mental health diagnoses are discussed with you and are determined with an inclusive mindset to support your cultural beliefs and influences.

Initial Assessment

An adequate history of your psychological, biological and social factors will be reviewed, as well as your presenting problem and what you hope to resolve.

Treatment Plan

We will formulate a treatment plan to accomplish your therapy goals. It will be theory-specific, unique to your needs, with a focus on resolving your presenting problem and reducing mental health symptoms.

Self-reporting tools for screening, diagnosing and monitoring your symptoms will be conducted throughout your treatment.

Resources & Referrals

Resources:  Books, articles, support groups, and other resources relevant to increasing your self-understanding will be offered to you throughout your treatment

Referrals:  If appropriate, referral(s) to other mental health treatment providers will be provided.
